What is myBeaumontChart?

myBeaumontChart offers you personalized and secure on-line access to portions of your medical record where Beaumont and your participating doctor store your health information. With myBeaumontChart, you can:

Is there a fee to use myBeaumontChart?

myBeaumontChart is a free service offered to our patients.

Is there an age restriction to use myBeaumontChart?

You must be 17 or older to use myBeaumontChart. Beaumont Health Systems does offer proxy access. To view more information, view the Proxy FAQ topic here.

How do I sign up?

You can sign up if you have a myBeaumontChart activation code provided to you during your office or hospital visit. This code will enable you to login and create your own user ID and password today. If you have an access code, please go here to sign up now. Please note: this activation code is only good for 60 days, so don't delay!

If you do not have an access code, Beaumont Health also offers an online access form! Information entered in this form will be compared to the information in our system, and if there is an exact match, you will gain instant access to your myBeaumontChart account. You can access this form by clicking here.

How do I know if my doctor participates in the myBeaumontChart program?

You can ask your doctor if they are using Beaumontís oneChart electronic medical record and if they are offering patient access to their chart.

What if my doctor is not a Beaumont doctor?

If your doctor is not a Beaumont doctor, you will not be able to access your doctor office visits through myBeaumontChart. However, if your outpatient tests were performed or processed at a Beaumont facility, you will see the results of those tests in your chart.

I declined a myBeaumontChart account in my physician office but now would like to have one. How can I request an account?

You can request an access code from the myBeaumontChart login page or click here to be taken directly to the page.

I received a letter declining my request for an account because by address was not correct. I called the number on the letter and they updated my record. Can I get an account now?

You can fill out our online access form and gain access today now that your information is correct. This form will check your information against the information in our system, and if it matches, you will gain instant access to your myBeaumontChart account. You can access this form by clicking here.

When can I see my test results in myBeaumontChart?

Your final test results are automatically released to your myBeaumontChart account immediately post discharge following an inpatient stay and 3 days after final resulting for outpatient activity. Sensitive results such as Anatomic Pathology results are automatically available to myBeaumontChart 3 business days after final resulting. Sensitive results such as Surgical Pathology and HIV results are automatically available to myBeaumontChart 7 days after final resulting. Physicians can manually release test results to the patientís myBeaumontChart account prior to the automatic release schedule.

I received a myBeaumontChart account and initially I could not see any of my past results or visits to the hospital or my physicianís office. Several months later I could, why is this?

Originally myBeaumontChart showed you data from the point of activation forward. Recently, we updated the system to show information prior to the account being created. myBeaumontChart is always being updated with new functionality in order to provide you with the information you need.

If some of my health information on myBeaumontChart is not correct, what should I do?

Your myBeaumontChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

If your doctor does not use oneChart in their office, these sections would be blank. If your doctor does use oneChart in their office and the information is not current, contact your doctorís office to update it.

Can I access my childís medical record within myBeaumontChart?

You can now view your child's medical information within your myBeaumontChart! You can ask your child's provider for proxy access to your child's records at your next clinic visit, or, if your child's provider doesn't use Epic in their office, you can request access online. To request access, click "Request an Access Code" from the myBeaumontChart login page, and then select "Request for child proxy access" and fill out this online request form. Once you have been granted proxy access, your child's record will appear in a separate tab within your myBeaumontChart, right next to yours.

I would like my spouse to have the ability to view my medical records within myBeaumontChart. How can I grant him/her access?

To allow your spouse, family member, or friend access to your medical records within myBeaumontChart, ask your provider. Your provider will provide you with an Authorization for Disclosure of Patient Medical Information form to sign. This form will allow your spouse, family member, or friend the ability to access your records within myBeaumontChart.

If your provider does not use Epic within their office, you can request this access from within your myBeaumontChart account. Once you have logged into your myBeaumontChart with your username and password, proceed to the menu item called "My Account" on the left hand, then select "My Family's Record". Within this page, look for the Adult Proxy Request hyperlink under "Who has access to my records?" to fill out a form to allow your loved one access to your medical information within their myBeaumontChart account.

I once had access to my childís myBeaumontChart information and now I do not. What happened?

Once your child turns 18, your access to their myBeaumontChart account is deactivated. Your (adult) child can now request his or her own myBeaumontChart account. If you would still like to have access to your (adult) child, have your (adult) child talk to his or her provider to allow you access, or have your (adult) child apply online through their own myBeaumontChart.

Can a legal guardian have access to a patientís myBeaumontChart account?

Yes, proxy access is given to court appointed legal guardians. To do this, provide your legal guardianship documents to your provider. Your provider will then give you access to the patient stated within these documents for the time frame outlined within these documents. After the legal guardianship documents have expired, your access to this patient's myBeaumontChart will also expire. To regain access, please provide the new documents to your provider.

I just moved here from out of state, and I have never been to a Beaumont before, but my child has. Can I still access my child's records via myBeaumontChart?

Yes, we can grant you non-patient proxy access. Ask your child's provider for access, or request it online. To request access, click "Request an Access Code" from the myBeaumontChart login page, and then select "Request for child proxy access" and fill out this online request form. You will be given a letter with an Activation Code to activate your myBeaumontChart account to view your child's records. Follow the instructions on this letter to gain access to your child's myBeaumontChart.

How is myBeaumontChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal id's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Futher, myBeaumontChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with myBeaumontChart. Unlike conventional e-mail, all myBeaumontChart messaging is done while you are securely logged on to our website.

What is your Privacy Policy?

myBeaumontChart is owned and operated by Beaumont Health Systems and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by myBeaumontChart.

I was logged out of myBeaumontChart, what happened?

We aim to protect your privacy and security of your information. While logged into myBeaumontChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of myBeaumontChart. We recommend that you log out of your myBeaumontChart session if you need to leave your computer for even a short period of time.

What do I need to use myBeaumontChart?

In order for the MyChart web site to work correctly, we recommend that you use one of the following web browsers with Session Cookies and Javascript enabled:

* Known issue: The Fasterfox and Fastestfox add-ons for Firefox are incompatible with myBeaumont Chart. These add-ons cause incorrect messages to open when viewing messages in your Inbox.

My access code does not work, what should I do?

For your security, your access code expires after 60 days and is no longer valid after the first time you use it. You can fill out our online access form by clicking here to gain access today if you information you enter exactly matches our patient records.

Is my access code my user ID?

No, your access code is not your myBeaumontChart ID or password. You will use this code only once to log into myBeaumontChart for the first time. (The code will expire after you have used it or after 60 days) When you log into myBeaumontChart the first time, you will then be asked to create your own unique myBeaumontChart ID and password.

Why am I not finding my other healthcare organization in MyChartCentral?

Organizations must participate with MyChartCentral in order for you to link to them through MyChartCentral.

Why isnít my health information the same at all of my healthcare organizations?

The information that each of your healthcare organizations knows about you might be different, depending on the type of care they give you and when you were last seen. You can help keep everyone up to date by organizing your records in Lucy and sharing your information among all the different places where you receive care.

What if I donít have multiple MyChart accounts?

Even if you have only one MyChart account, you can still use Lucy as your personal health record. MyChartCentral can provide access to your one MyChart account and Lucy with a single username and password.

Can I view my family members charts in MyChartCentral?

Yes, you can! MyChartCentral and Lucy let you see the same family members you can see here in myBeaumontChart. If youíd like to request access to a family memberís chart, please see Proxy Access topic in Frequently Asked Questions.

What should I do if the information I see in MyChart or Lucy is wrong?

If information in your myBeaumontChart account or Lucy is wrong, please contact your doctorís office to update it so your information can be corrected. If information from another healthcare organization is wrong, contact that organization to make sure your information is corrected. After the information looks right in MyChart, you can get an updated copy in Lucy by requesting your chart again.
