You can sign up if you have a myBeaumontChart activation code provided to you during your office or hospital visit. This code will enable you to login and create your own user ID and password today. Information entered in this form will be compared to the information in our system, and if there is an exact match, you will gain instant access to your myBeaumontChart account. You can ask your doctor if they are using Beaumont’s oneChart electronic medical record and if they are offering patient access to their chart.
If your doctor is not a Beaumont doctor, you will not be able to access your doctor office visits through myBeaumontChart. However, if your outpatient tests were performed or processed at a Beaumont facility, you will see the results of those tests in your chart.

You can fill out our online access form and gain access today now that your information is correct. This form will check your information against the information in our system, and if it matches, you will gain instant access to your myBeaumontChart account. Your final test results are automatically released to your myBeaumontChart account immediately post discharge following an inpatient stay and 3 days after final resulting for outpatient activity. Sensitive results such as Anatomic Pathology results are automatically available to myBeaumontChart 3 business days after final resulting. Sensitive results such as Surgical Pathology and HIV results are automatically available to myBeaumontChart 7 days after final resulting. Physicians can manually release test results to the patient’s myBeaumontChart account prior to the automatic release schedule.
Originally myBeaumontChart showed you data from the point of activation forward. Recently, we updated the system to show information prior to the account being created. myBeaumontChart is always being updated with new functionality in order to provide you with the information you need.
Your myBeaumontChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
If your doctor does not use oneChart in their office, these sections would be blank. If your doctor does use oneChart in their office and the information is not current, contact your doctor’s office to update it.
You can now view your child's medical information within your myBeaumontChart! You can ask your child's provider for proxy access to your child's records at your next clinic visit, or, if your child's provider doesn't use Epic in their office, you can request access online. To request access, click "Request an Access Code" from the myBeaumontChart login page, and then select "Request for child proxy access" and fill out this online request form. Once you have been granted proxy access, your child's record will appear in a separate tab within your myBeaumontChart, right next to yours.
To allow your spouse, family member, or friend access to your medical records within myBeaumontChart, ask your provider. Your provider will provide you with an Authorization for Disclosure of Patient Medical Information form to sign. This form will allow your spouse, family member, or friend the ability to access your records within myBeaumontChart.
If your provider does not use Epic within their office, you can request this access from within your myBeaumontChart account. Once you have logged into your myBeaumontChart with your username and password, proceed to the menu item called "My Account" on the left hand, then select "My Family's Record". Within this page, look for the Adult Proxy Request hyperlink under "Who has access to my records?" to fill out a form to allow your loved one access to your medical information within their myBeaumontChart account.
Once your child turns 18, your access to their myBeaumontChart account is deactivated. Your (adult) child can now request his or her own myBeaumontChart account. If you would still like to have access to your (adult) child, have your (adult) child talk to his or her provider to allow you access, or have your (adult) child apply online through their own myBeaumontChart.
Yes, proxy access is given to court appointed legal guardians. To do this, provide your legal guardianship documents to your provider. Your provider will then give you access to the patient stated within these documents for the time frame outlined within these documents. After the legal guardianship documents have expired, your access to this patient's myBeaumontChart will also expire. To regain access, please provide the new documents to your provider.
Yes, we can grant you non-patient proxy access. Ask your child's provider for access, or request it online. To request access, click "Request an Access Code" from the myBeaumontChart login page, and then select "Request for child proxy access" and fill out this online request form. You will be given a letter with an Activation Code to activate your myBeaumontChart account to view your child's records. Follow the instructions on this letter to gain access to your child's myBeaumontChart.

From the home page, click on the View your billing account summary quick link.
This will take you to your Billing Account Summary. From here, you can see the amount due for various accounts. Click the Pay button to make a payment, or click on the See Account Detail hyperlink to see more information about a particular account.
An E-Visit is a non-urgent, non-emergent online visit initiated by the patient. If your provider/primary clinic offers E-Visits you will see an option under the Messaging tab. Simply click "E-Visits" and you've started the workflow. You can select from a predefined list of reasons for your E-Visit. Then you'll be asked to verify pertinent information, before a problem-specific questionnaire is presented. Fill that out and your provider will receive your answers. Expect a response within 1 business day.
We currently are offering E-Visits at no charge to our patients. However, if insurance payers begin to reimburse this type of E-Visit we will change our no charge billing practice and add an E-Visit charge to our office billing practices. Any follow-up care ordered or directed as a result of the E-Visit, including, but not limited to, diagnostic tests, prescribed drugs, laboratory tests, referrals for subsequent care or an office visit, is the patient's financial responsibility.
Video visits offer you the ability to receive non-urgent care from your provider through a live video stream from your computer with a camera, Android, or iOS device. To schedule a video visit, simply log onto myBeaumontChart, go to the “Visits” menu, select “Schedule an Appointment” and choose the "Video Visit" option. From there, you will be prompted to provide the information necessary to schedule the video visit. Prior to your video visit appointment, test the device you are planning to use through the "Test Hardware" button. To join the video visit, log onto myBeaumontChart a few minutes prior to the scheduled appointment time and select the "Begin your video visit" banner.
Video visits will be offered at no cost during our pilot period, which extends through late spring 2016.
Currently, the following browsers are supported: Internet Explorer, Firefox, and Safari. Google Chrome is currently not supported. If you schedule a Video Visit, please use the "Test Hardware" feature to ensure your setup is compatible.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal id's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Futher, myBeaumontChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with myBeaumontChart. Unlike conventional e-mail, all myBeaumontChart messaging is done while you are securely logged on to our website.
myBeaumontChart is owned and operated by Beaumont Health Systems and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by myBeaumontChart.
We aim to protect your privacy and security of your information. While logged into myBeaumontChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of myBeaumontChart. We recommend that you log out of your myBeaumontChart session if you need to leave your computer for even a short period of time.
For your security, your access code expires after 60 days and is no longer valid after the first time you use it. You can fill out our online access form by clicking here to gain access today if you information you enter exactly matches our patient records.
No, your access code is not your myBeaumontChart ID or password. You will use this code only once to log into myBeaumontChart for the first time. (The code will expire after you have used it or after 60 days) When you log into myBeaumontChart the first time, you will then be asked to create your own unique myBeaumontChart ID and password.
MyChartCentral provides you with a central access point for all your MyChart accounts where you can access all accounts using a single username and password.
Lucy is a personal health record that gives you a permanent home to organize all your medical information, regardless of where it came from. You can request an updated copy of your Epic medical record at any time and store it in Lucy.
You can also add personal information about your health and choose to share it with Beaumont Health System and other organizations where you receive care. If your insurance changes or you move away and need to leave your current organization, Lucy will follow you.
Organizations must participate with MyChartCentral in order for you to link to them through MyChartCentral.
The information that each of your healthcare organizations knows about you might be different, depending on the type of care they give you and when you were last seen. You can help keep everyone up to date by organizing your records in Lucy and sharing your information among all the different places where you receive care.
Even if you have only one MyChart account, you can still use Lucy as your personal health record. MyChartCentral can provide access to your one MyChart account and Lucy with a single username and password.
Yes, you can! MyChartCentral and Lucy let you see the same family members you can see here in myBeaumontChart. If you’d like to request access to a family member’s chart, please see Proxy Access topic in Frequently Asked Questions.
If information in your myBeaumontChart account or Lucy is wrong, please contact your doctor’s office to update it so your information can be corrected. If information from another healthcare organization is wrong, contact that organization to make sure your information is corrected. After the information looks right in MyChart, you can get an updated copy in Lucy by requesting your chart again.
